Service users may have learning disabilities such as Asperger’s Syndrome, Autism, Down’s Syndrome or other non-specified, learning difficulties. We understand everybody is different, and due to their diagnosis, we recognise that some may have behavioural and emotional difficulties in addition to facing the challenges which come with having mental health and learning disabilities. For this reason, we provide an environment which is both safe and culturally sensitive, which will enable the service users to address issues pertinent to their specific needs as reflected in their care support plans.

We work as a team and for us, the concept of a team incorporates working with professionals, family members, carers and others. that are involved in caring for the service user.
Personalized Care Plans
We operate and are geared towards working with the service user, using the organisations, assessment, care planning and review system to highlight the areas for working with the service user, specifically the key targets or priorities.
Staff Team
We have a highly skilled staff team who are trained to assist those we work with. All staff are CRB, and DBS checked and go through a thorough screening process with adequate checks made to ensure suitability.

We have two loactaions where we offer our trusted care and support services which are both set within warm and friendly environments and are a place service users can call home.

Our Referral and Assessment Process

We place each individual at the heart of their care treating them with respect and involving
them in their care. Our assessment process is uniquely tailored and personal centred.                 A highly detailed care planning system is operated on a thorough pre-placement assessment.

1. Referral made to Gims Care Supported Living team
2. Assessment arranged and undertaken by our placement team within 2 days.
Typically, the home manager will attend the placement.
3. Feedback provided on whether our services can meet the needs of the service users
within 2 working days. We produce a therapeutic care pathway that accompanies
placement offer, and this will also include the level of staff support required.
4. Assessment pack formulated including care plans/risk management plan and funding
information.
5. Admission agreed and plans for transition arranged with referring team following
confirmation of acceptance of placement.
